Pathophysiological Mechanisms

T3 is the definitive metabolic hormone, binding nuclear thyroid hormone receptors (TR-alpha, TR-beta) with 10-fold higher affinity than T4. Total T3 equilibrium is governed by the concentration of circulating TBG and albumin. Oral estrogens, pregnancy, and SERMs increase hepatic TBG synthesis, artificially elevating Total T3 while Free T3 remains low or normal.

Scenario 01Elevated Total T3 (210 ng/dL) + Low-Normal Free T3 (2.6 pg/mL)

Significance: Elevated thyroxine-binding globulin (TBG) secondary to oral birth control, hormone replacement therapy, or pregnancy. Tissue hypothyroidism may persist.

Recommended Action: Base thyroid replacement dosing on Free T3 and Free T4 rather than Total T3; evaluate route of estrogen administration.
Scenario 02Normal Total T3 (120 ng/dL) + Low Free T3 (2.1 pg/mL)

Significance: Impaired peripheral 5'-deiodinase (DIO1/DIO2) conversion, high systemic cortisol, or systemic inflammation (Euthyroid Sick Syndrome).

Recommended Action: Check reverse T3, hs-CRP, serum ferritin, and selenium status; address physiological stressors and caloric restriction.
