Molecular Mechanism & Clinical Purpose

Dietary polyglutamates are hydrolyzed to monoglutamates in jejunal brush borders and converted to active 5-MTHF by dihydrofolate reductase and MTHFR. Transfers one-carbon units for DNA synthesis and repair.

Serum Folate (Vitamin B9) (Folate) functions as an indispensable enzymatic cofactor, antioxidant substrate, or nuclear transcription factor ligand. Cellular utilization depends upon intestinal active transport proteins, carrier-mediated plasma transport, and intracellular phosphorylation or conversion into active coenzyme forms. Inadequate tissue concentrations impair mitochondrial ATP generation, DNA repair mechanisms, neurotransmitter synthesis, and genomic methylation.

Pre-Analytical Considerations:

Protect light-sensitive specimens (such as Vitamin B6, B12, and Folate) from ultraviolet light exposure by using amber collection tubes.
