ALT vs. AST: The Biochemical Differences#

Understanding why ALT and AST elevate differently requires examining their cellular locations:

1. Alanine Aminotransferase (ALT / SGPT: Normal 7 to 56 U/L)#

  • High Liver Specificity: ALT is located almost exclusively within the cytoplasm of hepatocytes.
  • The Sensitive Indicator: Because it is concentrated in the liver, even mild liver inflammation causes ALT to leak into the bloodstream before symptoms appear.

2. Aspartate Aminotransferase (AST / SGOT: Normal 10 to 40 U/L)#

  • Low Liver Specificity: AST exists in both the cytoplasm and mitochondria of hepatocytes, but is also heavily expressed in skeletal muscle, cardiac muscle, kidneys, and red blood cells.
  • Non-Hepatic Elevations: Strenuous heavy weightlifting, running a marathon, or acute muscle trauma (rhabdomyolysis) can cause a dramatic spike in AST with normal or mildly elevated ALT.

The De Ritis Ratio (AST / ALT) Explained#

In 1957, Italian physician Fernando De Ritis demonstrated that the ratio of AST to ALT reveals the underlying mechanism and severity of liver disease:

Magnitude of Enzyme Elevation: What the Numbers Mean#

  • Mild Elevation (< 2 to 3× Upper Limit of Normal): The most common outpatient presentation. Typically caused by metabolic fatty liver disease, moderate alcohol intake, prescription medications, or intense physical exercise.
  • Moderate Elevation (3 to 10× Upper Limit of Normal): Seen in acute viral hepatitis exacerbations, active autoimmune hepatitis, or severe drug-induced liver injury.
  • Severe Acute Spikes (> 10 to 50× Upper Limit of Normal): Known as acute transaminitis. Seen in ischemic hepatitis ("shock liver"), severe acetaminophen overdose, acute hepatitis A/B infection, or acute toxic ingestion.
True Liver Function: Albumin, Bilirubin, and INR

Transaminases (ALT/AST) measure liver injury, not function. The true synthetic and excretory function of the liver is measured through Serum Albumin (protein synthesis), Total Bilirubin (bile clearance), and Prothrombin Time / INR (clotting factor synthesis).
