The physiological homeostasis of Serum Free Fatty Acids (Non-Esterified) (FFA) is governed by pancreatic islet beta-cell secretion, hepatic gluconeogenesis, skeletal muscle GLUT4 glucose transporter translocation, and peripheral insulin sensitivity. Cellular signal transduction occurs via the insulin receptor tyrosine kinase (IRTK), triggering phosphorylation of IRS-1 and IRS-2, which activates the phosphatidylinositol 3-kinase (PI3K)-Akt cascade and downregulates FOXO1 transcription factors. Impaired signaling causes systemic hyperinsulinemia, attenuated mitochondrial beta-oxidation, and accelerated hepatic de novo lipogenesis (DNL).
